It can be found in Act 2, after coming out of Seahaven, navigate to coordinates 53,53 on the area map. Once you enter the dungeon, take the teleporters in the following sequence:
  1. Left
  2. Middle
  3. Left
  4. Left
  5. Left
  6. Middle
  7. Right
